21 Partner Organizations Explored Building
Relationships Between the MACC Network and Health Care



Community based nonprofit organizations (CBOs) play a critical role in the health and well-being of our communities. Health and social stability are intertwined – up to 80% of long-term health outcomes and health related costs are tied to the Social Determinants of Health.

Over the past year, the MACC network brought together a diverse set of partners to develop a strategy for how a group of diverse CBOs might work together with health care systems to make our communities healthier and reduce the cost of care.

As a network, we can step into the health care arena with the scale we need to create balance at the table, and sustainably build the long-term relationships needed to make partnership work.

Throughout the fall and winter of 2018 our network
began building a relationship with health care partner Allina Health. We conducted a series of focus groups, facilitated dialogues between potential partners, and interviewed key stakeholders from all sides of the table.

Our conversations mined our shared expertise to uncover obstacles and opportunities, and identify the essential elements for building successful partnerships.

To apply our learnings MACC,
Allina Health, and Children's Minnesota decided to co-fund and co-create a pilot partnership, using referrals to explore how a relationship might work between health care providers and a network of community partners.

As we continue our work in 2019 and 2020 we're excited to pursue shared learning, and create space for innovative thinking about how networks like ours can work with health care partners.
